SCIENCE FAIR FORMS
ALL students beginning a science fair project must fill out paperwork PRIOR TO experimentation. Below, you will find the short form for grades 5-8, and the lengthy ISEF forms for grades 9-12. Some projects will require additional forms and signatures based on subject matter and risks. Click here for Human-Participant projects.
Again, ALL students will have forms to fill out that require appropriate signatures. These forms must be completed and signed BEFORE the student can move forward with the project. This is a matter of safety! Anyone who signs off on the form is stating that the project is appropriate and safe for the student to begin. That means that anyone signing the forms must know what the project is about, how the student will be conducting the measurements, if there are any risks involved, and make an informed decision on whether that student should continue. If there are concerns, those must be conveyed to the student and NO SIGNATURE should be given until the possible risks are mitigated.
These forms DO NOT register you for a science fair. Each fair will have its own registration process. You will receive registration instructions when you advance from each fair.

Elementary and Junior Division Form - Grades 5-8
All participants in grades 5-8, must complete this form and obtain necessary signatures to be considered for the Charter and Regional Science Fairs. This paperwork does not register you for a fair. Download and print the form. Complete the paperwork BEFORE experimentation begins (dates of signatures must be earlier than the start date of the project). This paperwork must be complete and turned into your teacher prior to attending the SLCharter Fair. Teachers will turn these in to the fair directors on the day of the fair.
Senior Division Online Forms - Grades 9-12
USEF requires that all senior division projects advancing to the Regional fair complete and UPLOAD the paperwork online by January 31. This is a lengthy process and we encourage you to start it in the fall, BEFORE knowing if your project will be advancing. Many of the forms and signatures need to be obtained prior to experimentation. Completing and UPLOADING this paperwork does not register you for a science fair, but must be done in order to advance.
